The Post Acute Care Management (PACM) Department is seeking a PTA or COTA to join our team as a PACM Liaison!
This is not a hands on patient care position however, as a Liaison you will use your skills as a PTA or COTA to evaluate patients for post acute rehab needs and help them get into one of our 3 skilled nursing facilities or our acute inpatient rehab.
The right candidate for this position will be a PTA or COTA with at least 2 years of experience preferably in the SNF or Rehab setting. You must be outgoing, detail oriented, and work well autonomously and with a team.
