Case Manager (Field Nurse)

Location: WK Medical Center Department: Hospice Classification: Full-Time Shift: 8:00 AM-5:00 PM Date Posted: 1/25/2023 Description: Willis-Knighton Hospice, part of the Home Care Department, is seeking a full time Registered Nurse to coordinate the care and manage patients in their home during the final stages of their life.  This is a visiting nurse position.

The ideal candidate will have a strong work ethic, excellent attendance, the ability to communicate effectively, and the passion to care for those who are at end of life. The Hospice nurse works with a caring, dedicated team to provide care and comfort which encompasses physical, emotional, social and spiritual needs. 

Strong critical thinking and clinical skills, dependability, and the ability to work both independently as well as with a team are essential.  A minimum of one year recent experience as a Registered Nurse in an acute care setting is required.  Proficient computer skills are necessary as patient documentation is done electronically.

Although office hours are 8:00-4:30, some flexibility in hours of work is essential to accommodate changing needs of our patients.  Extensive weekend and evening work is required during on-call duty.  Call is on a rotation basis with your team members.  Reliable transportation is vital and mileage is reimbursed.  A  general hospital orientation and a department specific orientation is provided.
