The right candidate will evaluate patients from adolescent through geriatric for appropriate post acute placement in the health system. Post acute areas include acute inpatient rehab, Extended Care Center (sub-acute rehab) and Progressive Care Center and Live Oak Skilled nursing facilities. Other duties include working with the PACM Liaisons to get insurance approval for admission, complete government required paperwork and move patients to the appropriate facility. This job requires extensive computer and phone work but clinical knowledge is key to helping our patients get what they need.
The right candidate for this challenging position must have at least an Associate Degree in Nursing and two years of clinical nursing experience and must also be self- motivated and team-oriented. An extensive orientation process is available to ensure a successful transition into this position.
